Tax Forms Now Available

The 2022 W-2 and 1095-C forms were either mailed or made available on Jan. 31. If you have any questions about your tax forms, please call the EmployeeCentral Contact Center at 855.475.4747, option 1.

If you opted to receive your tax forms online, here's how you can access them:

W-2 Form for CHI-Affiliated Employees:

1. Visit EmployeeCentral > Click 'View My W2' (under 'Payroll' in the 'Quick Links' section)
2. Under 'Year-End-Tax Statements', click 'Access Current Year End Statement' > Click '2022' (under 'Payroll' in the 'Quick Links' section)

(Please note: If you did not opt for electronic delivery of your W2 form, it will be available to view online starting today.)

W-2 Form for Dignity Health-Affiliated Employees: 

1. Visit EmployeeCentral > Click 'View My W2'
2. Once you're automatically taken to the ADP site, click 'Tax Statements' > Click 'View Statement'

(Please note: If you did not opt for electronic delivery of your W2 form, it will be available to view online starting today.)

1095-C form:

1. Visit MyBenefits > View and Update My Benefits > Click 'Go' > Click 'View My 1095-C'
