You must bring your child's immunization record and any letters from their school so we can determine which immunizations need to be given.
If able, please print forms and fill them out prior to your appointment time.
Please delay your visit if you, or a person in your home, are sick with fever, cough, sore throat, or shortness of breath or have had COVID-19 in the past two weeks.

(480) 728-2004All immunizations for children and adults are provided at no cost to those who are eligible.This includes children under age 19 who are covered by AHCCCS, those who do not have insurance, those whose insurance does not cover immunizations and American Indian/ Alaska Native children. Adults without insurance or whose insurance does not cover immunizations are also eligible.

The following is a list of pediatric/adolescent vaccines given at the clinics. If you are unsure whether your child is current, please ask your healthcare provider or bring your child's immunization record to one of our clinics for review. The vaccines that we may administer to children under 19 years old are: Adult vaccine availability- Chandler location only:
*Adult immunizations are only available at our Chandler Care Center location; however, adult flu immunizations are available at all locations.
Due to reaching our annual allotment of many adult vaccines, our current supply is very limited. At this time, we only have MMR and Tdap for adults. We will be receiving flu shots and other adult vaccines in October.
