Shared Learnings With Guest Speaker RaDonda Vaught

During National Patient Safety Week, CommonSpirit Health is hosting a special Shared Learning featuring RaDonda Vaught. RaDonda will explore the human factors and system issues that led to the fatal medication error for Charlene Murphey. During the session, we will explore medical error prevention and systems thinking, supporting our quest to promote patient safety and achieve zero harm.

This Shared Learning is for all CommonSpirit employees with a passion for error prevention and patient safety.

About Mrs. Vaught

RaDonda Vaught was the subject of one of the most high profile health care criminal cases in recent years. While working as a BSN prepared registered nurse, Mrs. Vaught committed a medication error that ended the life of a patient in 2017.

She was charged under administrative law by the Tennessee Department of Health, leading to the revocation of her nursing license by the Tennessee Board of Nursing in a July 2021 hearing. Additionally, she was charged under criminal law and found guilty by jury trial of two felony charges in March of 2022: Negligent Homicide, and Abuse of an Impaired Adult, with placement on an Elder Abuse registry in the state of Tennessee.

Mrs. Vaught is uniquely qualified to speak firsthand on the impact this sentinel event has had on her life and her profession, along with the implications of the legal actions that followed. A passionate advocate for safety and improvement, her story will be one that is not easily forgotten.

Test Internet Speed* a minimum of 25 Mbps Download speed is required.
To view a single HD Stream, a minimum of 6 Mbps Download speed is required. These numbers do NOT factor in any other traffic on the same network. Any other traffic will also consume your bandwidth, here are some example minimum bandwidth consumptions:

  • 4K Stream: 13 Mbps
  • HD Stream: 6 Mbps
  • Video Calling (Zoom, Skype, Facetime): 6 Mbps
  • Streaming Music: 1 Mbps
  • Email & Web Surfing: 600 Kbps
Bandwidth use information: According to the FCC, two or more connected devices and moderate to heavy internet use is recommended to have at least 12 megabits per second (Mbps) of download speed.
  • For four or more devices, 26 Mbps is recommended.
  • For two or more users who regularly stream high definition or 4K video, use videoconferencing, participate in online gaming, or work from home, a good internet speed of at least 12 to 26 Mbps is the minimum.
  • Homes with four or more people using four or more connected devices will most likely require a minimum download speed of more than 60 Mbps. We recommend even higher speeds – at least 100 Mbps – for households with heavy internet use.
